You need to take a physical examination or apply for a verification certificate of physical examination record after arriving in China. The application procedure may take 1-3 work days.

Please note: Take the physical test on an empty stomach. Bring along your passport and two 4.8×3.3cm color passport photos (please make sure the photos are in the appropriate size).

Physical examination checklist:

1.Receipt of appointment.

2.ID: passport + a copy of passport (photo page + visa page/visa-free page + amendments page)

3.Materials for the physical examination certificate application:

(1) Physical Examination Record for Foreigners, in either Chinese or English, valid for six months. The patient’s photo (with the hospital seal), the physical examination report, the signature of the doctor, and the hospital seal (which should be the same as the one on the patient’s photo), should be included in the document.

(2) The physical examination items should be based on the “Certificate of Health Examination For International Traveler”.

(3) The physical examination and report need to be issued by local public hospitals. A notary is necessary if the report is issued by private hospitals or clinics.

(4) If the physical examination report need to be approved by the Chinese Consulate in foreign countries, one should provide a certificate or certificate label from the Consulate.

(5) Two 2-inch color passport photos took in the last 6 months.

If there are missing documents or items, the consulting doctor may add items correspondingly to meet the verification requirements.
